I have to write a paper for my informatics class solving a clinical practice issues and I mainly do agency nursing right now, subbing for schools, private duty, home care but I don't have really have the kind of issues that they seem to want me to come up with. The current ideas that I was presented with in the class were: patient teaching, staff communication, shift report, patient prescriptions, medication administration, infection control. I tend to have some issues with patient teaching which I think could be solved by having access to an interactive patient portal, but I would really like to hear more about what field you are in, what your issues is and briefly if you think there is a technical way (smartphone, patient portal etc ) to solve it. I have an idea of where I want to go but I don't know if I will be able to find any supporting journal articles.

Med reconciliation not done upon discharge, or in the ED.
Then the patient is readmitted with a very outdated list of home meds.
The bedside nurse who finally does it has to deal with condescension from the patient because of this inaccurate med
list. More importantly, it is very dangerous to not update the med list when the patient is discharged.
There should be a hard stop for charting until basic information is entered.

Do you already use electronic health records? If so, is there not a place to do this, or does it not prompt you to do this?
Forgive my ignorance as I do not work with them. But I will in my new job!
