Hi everyone outthere, this is my first post and I need some help on a question. what can I write as an assesssment for incontinence on a patient that suffers from parkinsons, had a cva and has pneumonia? He is over 90 yrs old.

I am not real sure what you are asking.I would just state:client incont of b&b related to disease process.Requires q1-2 hours checks and frequent turning.I would also order preventive skin care orders if he doesn't have any break down.such as instruction to wash peri area after each incont episode and to keep client turned and repositioned at least hourly.

If you are ever in doubt, just write what you observe...what do you see? what does the patient do? what can/can't he do? what does he say? What's the result of you physical exam? and if your are really unsure, in additon you could write "pt (or family member) states that pt is positive for h/o cva, parkinson's and pneumonia"
