syntocinon

Just wondering what everyone else is doing in their practice in regards to giving syntocinon for third stage. I was taught to give it after the baby was born and the cord was clamped. But i'm working in a private sector and the obstertician likes it to be given once the head has birthed.

We give it after baby and placenta delivered in general. One doctor was doing a lot of research that supported doing this after baby cord clamped, and prior to placenta......so with her, it is given after birth of baby. It does hasten birth of the placenta but of course it would, causing the uterus to clamp down as it does. No problems seen doing it this way.

here in the uk, we give syntometrine (oxytocin and ergometrine) im after the birth, but if the mother desires she may have a physiological third stage with no injection at all, if all is normal. in addition, we are starting to do more delayed cord clamping, and this can be with or without the mum having had the injection.

U.S. ALSO standards encourage admin of oxytocin after delivery of anterior shoulder for history of maternal hemorrhage, concern about possible dystocia, multips or for any other concerns about possible uterine atony.

Most of our MD's ask for it after delivery of placenta but will frequently take a hint from the RN about starting it after anterior shoulder.

to the op; i think i'd rather have a shoulder safely emerging or out before giving oxytocin. imagine an unsuspected shoulder dystocia following the head? nah, too risky! there's no big hurry, and as mentioned by others, is often given with delivery of the placenta. in fact, it doesn't have to be routine in the first place!

We generally do it after the anterior shoulder has been delivered, either 10 units IM or 5 units IM and 5 units IV push
