Sample PIE Note

Help! I have to do my note this semester at clinical in PIE format. I understand the concept, but I need a visual example. I can't find one anywhere on the internet and my teacher did not hand one out.

Can someone help me out here?

Well, this goes back a few years to an old employer I had. You're talking about Problem, Intervention and Evaluation, is that right? We used this before the days of nursing diagnosis. At least that's how I'm remembering it.

An example would be:

P
- Nausea. "I feel like I'm going to throw up."

I
- Abdomen rounded and soft with bowel sounds all four quadrants. Remains NPO. IV fluids infusing at 100cc/hr. Medicated with Compazine 10mg IM for the nausea at 10am. Encouraged to lie on left side with legs flexed for comfort.

E
- At 10:30am patient was observed resting quietly. Voiced relief of nausea.
