For retaking nclex, do we really need to have our school again do the Nursing Program Verification form? It says in the form: this form is to be completed if accommodation(s) to testing procedures were granted to this candidate during their nursing program. However, if this form is not used, all of the information requested must be provided on original letterhead stationery of the nursing program.
My classmate insists that your transcript of records has to be sent again by the school together with this form. My cousin says she was given eligibility to take the exam without it, she just sent the request for reapply/repeat exam with the $75.
I'm trying to understand the "if this form is not used" part. I previously have submitted all their info requested (transcript of records, etc) with original school letterhead the first time I applied. I'm confused! Should I go back to my school again and have them send this form?

As a general rule no you do not have to send them again, when resitting the BON just needs paperwork and any fees

Accomodations for testing procedures refers to someone that has a disability and needs additional resources to write the exam. It has nothing to do with a retake of the exam.
You do not need to resubmit your transcripts if the state already has them, there is no mention of that anywhere here. You just need to pay the retest fee to the state and submit whatever application that they want, and the fee to Pearson-Vue.
