Report sheet

Taking first report of clinicals this Thursday, looking for good examples that I can use or draw from to make my own. Thanks in advance.

Sorry! I was playing around with the program and I accidentally cleared the file out and can't retrieve it.

Daytonite - I really like your report sheet. I'm going to make some minor changes based on a few things we do and use it for the rest of my preceptorship.

Knock yourself out! It was done on Word using Table commands. If you enable the gridlines, you'll see the cells and how they are formatted.
