I've just been working in Hospice two weeks now. The book that everyone raves about is the one mentioned by River - called 'Notes on Symptom Control in Hospice & Palliative Care' by Peter Kaye. It is not out of print - you can order the 2005 (latest) edition for $35. plus $5. S&H from:
