Recommendation letter

I asked my supervisor for a recommendation letter to get into nursing school, and he said write one and I'll sign it. I've never written any type of letter of recommendation so I have no idea what to say, and I feel like it's hard to recommend yourself. Anyone have a template or written one that could give me some advice?

    That's strange that your supervisor would ask you to write your own recommendation letter, is there anyone else you could ask?

    It is strange but I understand it, he works 2 jobs has kids so he's very busy and doesn't have time to sit down and do it I guess.
