well i think in your primary residence you should put where you live... the philippines....

the rest is pretty easy to fill out... you just write in the info they requested and send it off...

i got a little confuse with your use of articles. do you mean you are writing to the state of new mexico's bon? or is it you are currently in new mexico writing that state's bon?

im confused regarding the re-examination process..

according to the instruction here in my received document, i should re-apply for the exam through online or by phone... but i have received an nclex examination registration form to be filled up... and another form titled as reapplication for rn/lpn licensing examination...

im done filing up the nclex examination registration form, but how about the reapplication for rn/lpn licensing examination form...

what should i write in the ff?

nm bon file #_____________ your reference numbe from board

date org exam app. submitted to board __________________ the firt date that ou got approval from board for instance if our approval was jan 1st thats the date they want

date registered with pearson vue____________ (is it the online thing or the form to be mailed) the date ou applied to pv to write our exam rememebr you are given a time frame.

pv candidate id #______________ (attach copy of current pv reg?)

hope this help just a quick question when did ou write the exam and how many days did they say you can resit. is it 45 or 90 days.
