Re-applying for Nursing school

I have decided to re-apply to Nursing School. I left because I was burnt out. I worked around 16 hours and my sleeping habits were odd (I posted my situation in other thread).

I have to fill out this supplementary form to provide an explanation. Would someone give advise if I am going in right direction?

Are there factors that you believe are important/pertinent to the consideration of your application(s), especially in our review

of your academic performance at the secondary and/or post-secondary level? If yes, please explain.

In this section, I wrote that there are 2 factors that are important in consideration of my application. First one was the amount of work hours I worked per week and secondly, my sleeping habits and how I was unable to focus properly during school time because of lack of sleep. I then explained how it affected my studies negatively. (I withdrew from the program before I could have an academic penalty, I do have 2 courses that I did not drop with gpa of 3.8)

Question #2If you are a current/previous student seeking readmission/reinstatement, please provide an outline of

how you plan to achieve academic success upon re-entering the program

In this section, I explained that I will reduce my work hours per week. Change my sleeping habits, if unable to, I will seek medical assistance. furthermore, I will begin to use calender/agenda to write important dates such as test days and assignment due dates.

I honestly don't know what else to write for question #2, it seems short.

Any suggestions?

You have to give clear examples of changes you have made now that will help you succeed in nursing school.

Maybe you could write how your going to change your sleeping habits and work hours. Hope this helps.
