Radial Hematomas

What is the best way to deal with radial hematomas, we are still seeing quite high rates with the TR band. What sort of rates does everyone see and how do you prevent and deal with it.

Most of the time I allow the band to sit an extra 15-20 minutes before I start to release air. When I do start to release it, if I see even a small bit of shadowing, I stop and leave it on a little longer (starting over with my initial count). There has only been one time I've seen someone bleeding after using this method and that was because the pt was going against the medical advice by moving furniture around the room, lifting stuff in the room eager to be getting discharged.  They had to stay longer of course because they were bleeding like crazy.
