Can anyone answer these questions, I'm studying for a medication calculation test and I feel dumb but I can't find the anwers.

1. ) Tom is taking Dilantin 300 mg at bedtime. He is unable to swallow the capsules, so you are asked to give the suspension instead. You say:

a) no problem

b) the injection should be used

c) only if the doctor writes an order

d.) once i clarify the dose

2.) Levothyroxine (Synthroid) 0.1 mg po daily is to be taken for Tom. The doctor changes all his meds to the IV route. what is the corresponding IV dose of levothyroxine?

a.)0.05 mg

b.)0.1 mg

c.)0.2 mg

d.) 1 mg

the answer i would give is c.

this is from the pdr monograph for phenytoin http://www.drugs.com/pdr/phenytoin.html - read the first paragraph under the heading of "dosage and administration" which states "because there is approximately an 8% increase in drug content with the free acid form [the suspension] over that of the sodium salt [the oral capsule], dosage adjustments and serum level monitoring may be necessary when switching from a product formulated with the free acid to a product formulated with the sodium salt and vice versa."

the answer i would give is a, half the oral dose. this is according to my drug handbook, mosby's nursing drug handbook.
