At my job the staff was documenting on the back of the MAR for PRNs when our boss showed me a page right next to it and said this is where we are really suppose to document. I rewrote everything I had previously written onto the page she said we are suppose to use but now I am worried I may have messed up because I did what my CNA instructor told us to do when faced with errors in the MAR and that is to draw a line through, put in ( )'s the change/reason for change/error and initial and circle your initial. What I was doing basically was transferring previous documentation over into the proper area so should I have written "accidentally wrote in wrong area" or "rewrote in proper area"? Which is better? And I'm concerned about the strike through as I am reading that we are never suppose to strike a line through or white out/cross out documentation.
