This is the medication guide from Affinity. I hope it helps! MedicationGuide1.7.pdf
  2. I'm on the second year! What about you?
  3. Im my case I get tested twice a month. Usually the first week of the month and then on the 20th's (21st-28th) On some occasions they have thrown a blood test in between,(more specialized and expensive testing), but when that happens that blood test is actually replacing an urine test so it still 2 test in within that month.
  4. My understanding is that you can provide your monitoring program and the BON your work schedule. They should be able to avoid "randomly" select you on the days you work. I was told that this "random" test are "schedule in advance". Meaning that they (The BON-Monitoring Agency) knows in advance when are you getting tested. You are the one that doesn't know. It's like they create a calendar in advance with the dates and the different type of test you will be doing for the year. It's "random" for you, not them. I hope this helps! Give them a call. If you keep an open communication with them you should be fine. Edited to add: I am not sure of every state rules. But where I live (PA) my monitoring program (Recovery Trek) offers a home kit test. It's called "PROOF". I believe Affinity has something similar as well. I use Recovery Trek for my testing check-ins and Affinity for the rest (NA Meeting check-ins, program fee payments, and reports). You can use these "Home Kits" for special circumstances like being selected on a working day or being out of state for emergencies or things of that nature.

  6. April Thompson you are openly admitting that you diverted from work. This should tell you that obviously you have an addiction. Base on your following statement this is what I can understand; "I'm having trouble deciding if I'm willing to do the program or if I'm going to surrender my license, I've been on pain management for basically 10 years and I'm terrified to cold turkey stop gabapentin and hydrocodone" It seems like you are not ready to admit that you are an addict and that you are willing to take all the necessary steps to get better and recover. All I can get from your statement is that you are more concern on wether you can continue to use, instead of worrying about losing the license you worked so hard for. My advise to you is that you take responsibility for your actions and go into the program which is a privilege and an opportunity for you to recover and continue to be a nurse. It's not going to be easy but in my experience getting caught diverting and getting into the program was the best thing that happened to me. I wish you luck!
