  1. I work in home iv infusion and we draw troughs 30 min. to just prior to hanging the med., peaks are drawn 30 min to 1 hour. According to our pharmacists as long as you document the exact time the previous dose was given, time of the trough, time drug started and completed they can calculate a more accurate dosage. A bun and creatinine should drawn at least once a week as well.
  2. Infusaports should be flushed once a month. You could attempt to flush as long as you don't meet any resistance. I have encountered ports that have remained patent beyond the 4 weeks. Hoped this helped.
  3. I work for a home infusion company and the nurses place them in the home. They are either x-rayed by a mobile unit or the patient is sent to the hospital.
