  1. When reviewing the notes from the previous shift I realized that I had read a patient's note before. I went back over previous documentation and saw that the nurse I was relieving cut/ pasted a note written days before about a different patient. The note was fairly generic and actually seemed likely to be accurate in most descriptions (took meds as prescribed, pain level, etc) but it was definitely cut and pasted. I'm not a supervisor, Should I mention this to her and warn her that it's not appropriate nor ethical? Or, report her? I should add, she's a fairly new nurse but everything I've seen is that she's a good at her job: responsible, kind with patients, and helpful with colleagues; she's also a single mom with a four-year-old and twin daughters who are two. I would hate to see her get fired... but I'm really torn as what to do.
  2. Not sure if the details are well known, but it's more complicated than what has been made public by most news outlets. There is a great article from the NIH about this case. This could have happened to most any/ all of us. Search for: Reconsidering the application of systems thinking in healthcare: the RaDonda Vaught case - PMC (nih.gov)
  3. I would steer clear of Western Govenors University. I recently applied, jumped through the usual admission hoops and was admitted and scheduled to start classes October 1 (two weeks away). Online orientation is the final step to be completed before beginning classes. In the second module of the online orientation you're required to enter information that will be added to your email signature... the only problem is, they have not connected me to their email system. I have called IT twice and contacted them again from my personal email. They wrote back giving me my non-functioning email address. I contacted my admission counselor who told me to contact IT. I contacted my mentor (who WGU describes as your go-to contact person). She told me that I could postpone my admission and start classes in November (after I changed my work schedule to accommodate my class schedule). If they can't give you access to an email account, what other deficiencies might they have that will prevent you from finishing your degree (after you spend thousands in tuition). No thank you.
