  1. I am stuck on a question about cost effective care. I have several of my books with me but none explain it. What in the following scenarios constitutes cost effective care to a patient? 1. Providing palliative care to a terminally ill client. 2. Beginning discharge planning on admit. 3. Counseling clients on cigarette smoking cessation. 4. Educating a group of parents on the importance of childhood immunizations. 5. Performing a postop wound dressing change using clean gloves. From what I kind of understand, 3 and 4 are cost effective, right? Because they are preventive measures that would hopefully prevent health complications which would prevent more money being spent on medical care... I think 2 may be cost effective because a plan is being put in motion to help the patient get out of the hospital and hopefully keep the patient from another admit if planning is adequate. I am saying no to 1 because they are already sick and all they can get is comfort care. 5, I am unsure. Clean gloves obviously will prevent as much contamination as possible... ??? Help!

  4. A client is admitted to the intensive care unit after overdosing on meperidine. What is the nurse's first priority? 1. Maintain continuous cardiac monitoring. 2. Administer naloxone hydrochloride 0.4 mg IV every 2-3 minutes prn. 3. Provide alprazolam 0.25 mg PO PRN. 4. Initiate intravenous fluid resuscitation with lactated ringers at 125 mL/hr. Ok, I need help with this one. I figure if the patient is going to ICU, the ER would have given the patient Naloxone. I have eliminated 3 because that is for benzo OD. Now I am stuck

  6. I need a little help on a question... The patient is a 13 year old who presented to the ER unconscious with rapid breathing. His potassium is 5.2,glucose 420and is positive for ketones. He is in metabolic acidosis. I'm thinking I would give him Kayexalate to decrease the potassium. ? Start a second line for fluid resuscitation. I am slightly sure those are actions I would take.. But would I Initiate IV of NS 500 ml with Regular insulin 60 units at 50 mL/hr, give 2 L O2, and start a foley as well?
  7. I have had tough instructors and I have seen a few who clearly didn't like particular students... However, I have never felt like any of them wanted to see anyone fail. In my experience I have had a few go the extra mile to help me succeed when I was struggling. It actually made me cry to know they cared that much.
