can someone help me with the importance of nursing documentation and impact it has on patient care

I will try and make it clearer, you are looking after a patient and during the process of care you noticed that the patient is a high risk fall, what action and evaluation would you put in place. I understand that this patient might need a medication review. the episode of care in this contest is that you were walking a patient you are caring for and notice he is a high risk of fall. what do you write to make a 1000 word out of this episode.
