I asked my DON the same question last week after this same incident happened on my shift and she told me this ..."Please remember the possibility of unseen injuries".. For example, the res may be on the floor, on the mat, from a bed in lowest position, BUT what if she hit her head on the bedside table and has an internal bleed that poses problems later? I had rather fill out the report and be safe AND avoid any possible guilt of being at fault if something did happen.

I always give my cna's report. Like others who have responded, I feel that they cannot be of much help to me unless they know what is going on. I have nurse co-workers that tell me that it is none of the cna's business and that nurses "should know more", I personally think this is a bunch of crap because cna's are our eyes and ears and they are more willing to help us if we involve them a little, they don't have to know the residents full history, just what is going on with them that they can help out with. I have been told that they respect me more due to the simple fact of including them and not outcasting them like I am so much more superior.

Finally!! I talked to my DON.... I will be working med cart about 3 days a week ,7-3, assisting with care plans, assisting with MDS, and mainly I will be doing restorative nursing..(Weights, meals, ROM & etc...). However, I will be losing my $1.00/hr shift diff which I feel is unfair because this job is much more than what I do now.. I just don't know!!??!!??

Well, I don't really know all the details yet. I am supposed to meet with the DON Monday morning to talk about it & I will let you all know. What I posted was all I have been told so far but like you I think it sounds like a lot. I do know that this job was being done by 2 people but they are combining it to one job. The more I think about it the more i think it is not for me. I will know more in a few days & I will let you know an update. Thanks so much!!

Hi everyone, I need your input. I graduated from LPN school just 6 months ago and I have been working in a LTC facility since. I am very happy with the facility but not the shift. I work 11-7 and my hubby also works this shift, therefore we have to carry my two babies to stay away from home every single night which is a complete nightmare!!! I told my DON I was interested in days and today I was confronted with an offer- It will be full time 7-3, I will be working a med cart, restorative nursing, writing care plans for all residents and working with MDS. This sounds like a lot to me and I haven't been told yet but I figure there will be a pay cut due to shift difference I am getting now. On my night shift I currently work I just do basic nursing and work a med cart. I want to make things easy for my family but the job description alone stresses me out probably because I am still a "new graduate" I would just like to know what some of you think. I don't expect you to tell me what to do just give me some input---much appreciated. Thanks!!

Thanks to you all for the replies.. it is FINALLY over.. It was not as bad as I had expected it to be. They only went into one residents room with me and stayed all day with two of my co-workers. I don't know if that is good or bad but it helped my stress level. I was stopped several times in the hallways and asked questions, mostly common sense ones. While they were there they were mostly on charts, MARS & so forth. So glad it is over!!! Thanks LeAnn

Thank you for the info. Yes, the patient nurse ratio is ridiculous and we will probably be written up for poor staffing if nothing else. I am just planning to try to be confidient and follow all the rule I learned in school.. Thanks for your advice!!! LeAnn

HI, I am a recent LPN graduate working on a temporary license at a local nursing home. I have been there about a month. Anyway, I just completed my orientation and got placed on a med cart alone 2 days ago. I was off today and I was called and told that state had walked in to do the survey and that they would be there tomorrow also, (which I work).. frankly I am terrified, I do not know what to expect and cannot get anyone I work with to tell me much either. The only thing I was told was that an RN would go with me to do invasive procedures to follow the legalities of my temp license. I really do not know what is and is not acceptable to the state as far as med pass because I have had a really shotty orientation. The only thing I have been told is to be sure I was through with med pass in the one hour time frame, which is impossible for me because I have 74 residents and patient safety seems more important to me.. anyway, I am sorry this was soo long but I desparately need some advice!!!
