Hello nurses! Before i ask, i just passed my NCLEX last march under hawaii then I moved to MA and processing my reciprocity i thought I'm done dealing with CGFNS. I should have known that everytime you moved to another state you have to re-evaluate your CES report. I have jobs waiting for me but still waiting for CGFN CES repot to process my MA license. The CGFNS renew their forms i have to start from the scratch. Anyone who encountered the new form? Page.1 is it ok to leave a blank? current school or authority name where academic records/(if different name) or have to put N/A page.2 is it ok to leave blank? name of diploma/degree obtained in original language. Page.3 about the second page. Under subject areas number.1 community health concepts the school have to circle the independent or integrated work? And clinical education? Replies much appreciated
