  1. I'm doing a CP for a pt with Hepatic Encephalopathy. His ammonia levels were high and H&H low. I'm working on my psychosocial nursing dx. I wanted to do "Disturbed though process r/t high levels of serum ammonia AEB serum ammonia lab value 88 (18-45), client exhibiting slurred speech, forget fullness, unsteady gate, slow movements, drowsiness, and asking 4 times "when is lunch?" after being told when lunch was." I'm having trouble with my short term goal (we don't do long term). I was thinking "client will remain free of injury by end of shift." or "Client will not fall or injury self by end of shift." Since this is for psychosocial, I wasn't sure if these were appropriate goals. We do a physical, psychosocial, and educational dx per client. Since its a psychosocial dx, does the goal have to be a psychosocial thing? He didn't have any anxiety, fears, or ineffective coping. He was pretty positive and upbeat about his tx and prognosis. Any advice is appreciated. thank you!
  2. Yes definitely attend both days. And do your health requirements early. It can be a long process since you need titers and possibly vaccinations for various things and if you don't have proof of vaccinations/titers then you won't be able to go to the hospital. Lauren made a FB group early and it was helpful. FB is gana be your lifeline so for those who don't have a FB you better get into it!

  4. HI, I'm a 2nd semester at Pierce. The school does a bundle for the majority of books you need for 1st and 2nd semester that has most books and access codes for Internet based assignments. I suggest waiting for orientation. Also, they want you to have the most up to date books. Good luck everyone!

  6. I go to LAPC and scored in the 90 percentile overall. I got somewhere in the 60s for my science section! My prereqs were B's for anat/physio and a C for micro. LAPC is a lottery so you just have to get lucky. good luck! Oh and I only studied for a few days before my exam... and I was sick. It's not that hard. Just study the book. Good luck!

Functional Health Pattern: Cognitive-Perceptual Nursing Diagnosis: Knowledge deficit to changes in vision Related to: Lack of exposure to vision changes Can someone let me know if I'm on the right track or if my Nursing Dx should be on something else? This is the synopsis that was given: "Mrs. Margaret Cooper is a 70-year-old African American female, who has come to Eastridge Family Health Center (EFHC) for an annual physical examination. Mrs. Cooper is a widow who lives alone in a small second-floor apartment and is a retail sales clerk. EFHC admission nurse John Barton conducts a health interview with Mrs. Cooper and notes that Mrs. Cooper is alert and oriented, dressed appropriately for the weather, and answers questions readily and completely. "Mrs. Cooper states that she is allergic to dust mites and mold, and takes Claritin-D (10 mg loraidine/240 mg pseudoephedrine sulfate) every day. She reports that she has osteoarthritis in both hips and knees and takes 650 mg Tylenol (acetaminophen) every 6 hours. She also takes temazepam (Restoril) 15 mg at bedtime for sleep. "During the interview, Mrs. Cooper complains of being 'unable to see cars coming up alongside when driving,' but denies any eye pain. Mrs. Cooper reveals that she is experiencing "blind spots," which are darkening and increasing in size and number, and that she is losing visual acuity and is beginning to see 'halos' around lights. Mrs. Cooper's daughter has accompanied her to the clinic visit and notes that Mrs. Cooper has had several small fender bender car accidents in the last year. "John performs standard admissions assessments on Mrs. Cooper and notes that she has an absence of the red reflex in the right eye and that her Snellen chart reading is 20/40 (left), 20/50 (right), and 20/50 (both eyes).   10. Getting all this stuff done before orientation is so confusing! I was just barely able to register for classes this week - one step done! Has anyone done the health check at Pierce yet? For the white shoes - do they need to be 100% white? Or can they have a little bit of color. Sometimes the Nike white shoes may have a colored swoosh or something. Very excited to start soon!!
  11. I've been working on all of that - lots of stuff to get in order! Do we do all of the health check ups at Pierce or through our own PCP?
  12. Hi! Can't wait to start classes! Has anyone heard from Pierce about the packets? I emailed the other day and haven't gotten a response. Just making sure I haven't accidentally deleted anything.
  13. Got my acceptance today around noon! I was sure the email would say "sorry not accepted" because my grades weren't so great, but yay! Can't wait to start in Fall!
