  1. Every nurse has her own style on what they write for an admission note on a newborn. Some include a lot of details while others write just the basics. Im assuming the newborn is in well baby nursery and not NICU. I would look at other nurses notes on admission to get an idea of what they include in their note. This is an example: Newborn FT baby boy/girl, weight 3600 gm. admitted to WBN. Can include that you identified the baby with a particular RN. Active, alert, pink color. No respiratory distress noted. HR..., RR..., Temp. (Rectal/axillary)..., BP..... , Dxt..... No void/meconium passed. No visitors at ths time. Will continue to monitor.
