  1. the newborn had no complications so what can i write for the focus notes?
  2. There is 1 part in my assessment page where I put a narrative nurse's note for a mother who had a baby. then there's a part for the focused nurses note for the newborn. what is the focused nurses note??
  3. I assessed a mother in the postpartum floor. She has no prior medical history, no complications during labor, newborn was normal with no complications. She was in her PPD #1. She had normal vital signs and pain was a 0. She walks around often, eats regular food, voids, and has maximum interaction with her baby. She breastfeeds her son. I did assessments and didn't find any abnormality. She hasn't had a bowel movement yet and she knows to drink a lot of water, ambulate, and eat foods with fiber. I looked into the possible nursing diagnoses and fatigue wouldn't fit her because she said she does feel tired. Pain, I can't do because she reports of no pain. She breastfeeds which was effective. The only abnormality was her Hgb and Hct values which were low. Can I use THESE into nursing diagnoses even though I'm not using the actual assessments I did? I planned on doing risk for injury due to abnormal lab values.
  4. I had my clinical in a postpartum maternity floor. I had a mother who had her second child through lady partsl delivery. she didn't have any complications before or during her labor. when i assessed her, she was breastfeeding often and bonding with her son. her pain level was a 0 out of 10. fundus was fine, breasts soft, moderate lochia, perineum intact. no episotomy. nothing out of the ordinary. newborn is normal as well. the only thing was that in her labs her Hgb was low. It was a 8.8. Her Hct was low too. I heard this was normal for moms who are breastfeeding. she was only in her postpartum day 1 and didn't have a bowel movement yet. can i still write risk for constipation for this? also i didn't ask her if she sleeps at night, but i'm assuming she doesn't because she breastfeeds. so fatigue?
  5. i'm starting my ob clinical soon and it will be 2 days, 12 hours each. this will be my first time having a long clinical and i'm very nervous because it's OB. i think it's diff from the med surg floors. what will the students be doing? will we be actually assessing?
  6. I start my first day in maternity tomorrow. I will have 2 days of maternity for 12 hours each day and 2 days at Peds for 12 hours each day. I'm feeling very nervous because this will be my first all day clinical, other times I had it for 6 hours each day. I'm feeling anxious as I don't know what to expect since I've only been to med-surg floors. Can anyone please give me advice on what I should prepare ahead and know. What should I expect? I know we will each get a mother and assess, but I'm so nervous about assessing the mothers and babies. How would a 12 day rotation at the maternity floor be like? what should i bring to eat? or drink?
