Sounds like where I worked. 6-2 shift everything was always fine in report, by 2:30 when I did my assessments O2 sats were 79% on 2L but the charting indicates even unlabored breathing w/ 94% O2. Or a residents temp on AM shift was always 98.7 but when I took it they were 100.0+ this happened several days in a row. When I advised the DON on some of the "odd situations" of everything being fine on AM shift and not on the PM shift I was the one questioned. I was accused of 'looking through charts to find errors' of the previous nurse. REALLY? Of course I'm going to read previous entries, we have the same residents and that is what you do, read previous entries to see what has been happening! AND yes, I did get fired from my job--not the nurse who always indicated that residents were fine, that didn't do TX's "because she didn't feel like it" who didn't give residents their inhalers, sign out a PRN, and consistently made late entries or had other nurses do her documentation. Yes, they threw me under the bus!! Did I want to get fired? NO!! I was in the process of looking for another job when this happened so I guess they did me a favor by freeing up my time for my job search!

I would go with your initial 'gut' feeling. I also had a job interview so messed up I didn't even realize I was in an interview. The facility had lost some of my papers/couldn't find some of my papers and I knew in my gut that this place was disorganized but took the job anyway. I could never find the forms I needed etc. Bottom line is to go with your gut feeling!
