Need help with prioritizing
Hi I have been working per diem in ltac 6 mons now, 7-3p and I have 4-5 patients. I have a big issue I am always behind on med pass and I don't even have time to do wound treatments. Some issues that arise are a patient o2 sat are too low, elevated bp or low, anxious patients, trouble shooting Iv pump, I don't get to know my patients, just a quick glance at labs, vs and progress notes. Also I am frequently interrupted by family, PT/OT regarding pain meds and other things. Sometimes I am stuck with one patient that have an acute issue going on and I don't get to see my other patients in a timely manner. I feel so awful. I never take a lunch break, because I am so behind on my charting. Please I need suggestions on how to prioritize patient care and time-management Thanks

Pump rates verification
Hi All! The pharmacy sends iv med w the rates on them to be infused at. I would like to know how can you verify that a pump rate for a IV med is correct? For example Levofloxacin 500 mg in 100 ml (NS) to be infused at 75ml/hr. Thanks!!!

New grad too stressed about the job- Is this normal?
Me too I am also a new grad, its hard to get med pass on time, attending to acute issues, supervising aids, document in 8 hrs have (24 residents). Even when I do get a break I am so overwhelmed that I do some documenting. While the other nurses I work with always have time to chat w others, text, sit and finish w everything on time. I don't know how they do it.

Do you call the health proxy if they want to reverse the code status? Thanks, I would appreciate your input on this matter.

First med error
There should be enough nurses to provide adequte safe care, thats a big probably. Many Hosp, Ltc/Snf etc have toatally neglected pt safety by not having enough staff on the units. School teaches you the basics (perfect world), once you are out there on your own you have to learn to use minimal resources. Its a challenge for novice.

What will you do?
Hi all!! I am a new nurse and was faced with this problem: A MD orders ceftriaxone for a pt. However the pt does not know if they are allegeric to lidocaine (NKA, no dental procedures ever in the past ). Do you reconstitute this medication with normal saline or lidocaine? What are the consequences of giving a medication to a pt if they have NKA and they end up having a major allergry reaction? Thanks!

Clinic RN-new grad
health clinics working environment are laid back you mostly sit all day through out your shift. you will be doing telephone triaging, refill rx, fill out medication prior authorization forms for pts and administer vaccines. assist md/np with stitching, wound dressing changes etc. there are no change of shift reporting. a lot of places require experience, but as long as you demonstrate that you are enthusiastic, you desire to learn and want to take on a challenge you will be fine. thats how i got a job at a clinic as a new rn grad. good luck!!

Co-signature for Insulin administration
Hi All!! I am a new RN and hope to get job at a hospital or nursing rehab so I can build my nursing skills and confidence. I have heard rumors that some cna/pca dont check blood surgar of pts and just write a random number. What do you do if you suspect this? Should a nurse check the blood surgar just to be on the safe side for each pt that may need insulin? Also I would like to know do nurses co-sign to adminster insulin? What do you do if you are alone for example a school nurse or your alone on the unit? Thanks!

Hi! :) I am planning to apply to a couple of RN residency programs. But first I would like to hear from those that have completed or is currently in a RN residency program. Do they offer relocation assistance? Do they offer loan repayment assistance? What's the starting salary? Were you able to secure a job at the end of the program? Any information will be appreciated!
