Hi MAnewgrad2012, I just stumbled across the MGH new grad internship and was glad to see your posts! How is it going so far? I am sure you are busy with the program now that it has been 2 months since you have started. Today I tried to call and e-mail MGH to see if I could get any information about if the program is still running, but no response yet. I will be graduating in December and would love the opportunity to learn and gain experience at MGH. Please let me know any tips you have for getting into the program, and when I should be applying. Hello there! I have finished my clinical paperwork and just wanted some feedback before I turn it in. My patient was admitted to the hospital with a wound infection/sepsis resulting from where the patient had a femoropopliteal bypass graft done on 9-7. She was going in to another surgery on the day of my clinical (9-27) to clean the wound out and a possible redo of the bypass graft. I know that is not much information on my patient, but I was looking for feedback more on the mechanics of my diagnoses. The ones I picked are: 1. Acute pain r/t surgical intervention on left groin aeb patient reporting pain rated 7/10 on a VAS scale. 2. Ineffective protection r/t abnormal blood profiles, drug therapies, wound culture results, and treatments aeb RBC/HGB/HCT blood profiles low showing anemia, drug administration that alter clotting including heparin and aspirin, wound culture results positive for E. coli and P. mirabilis, and surgery scheduled on 9-27. 3. Powerlessness r/t illness-related regimen aeb patient sobbing, "Why did this have to happen to me? I just want this all to be over," in the holding room of the OR. I have a little trouble with getting the diagnoses just right. My teacher always has said that I do not word them correctly. I know the second one is very long... is it okay that I use that many etiologies and assessment data, or should I just try to use one? Any feedback would be appreciated!! Thank you so much for your help!!

Oh and one last thing... my teacher would prefer us not using "RISK FOR" nursing diagnoses...

Hello there! I had a patient at clinicals that I thought was a little hard to do the care plan for. Here is a little about the patient: Pt is a 43/female admitted because of right flank pain (which has been going on for 3 months) on a VAS scale of 7/10. CT scan showed a 16.4 cm cystic mass on the right kidney, pressing on the inferior margin of the liver. A needle biopsy revealed the mass was benign. Pt has a history of controlled hypertension, and family history is positive for diabetes, hypertension, and hypercholesterolemia. Pt also reported she is trying to get pregnant. The nursing diagnoses I chose are: 1. Acute pain r/t 16.4 cm cystic mass on right kidney a/e/b patient reporting right flank pain rated 7/10 on a VAS scale. 2. Imbalanced nutrition: less than the body requirements r/t food intake less than the RDA a/e/b patient eating less than 50% of all meals. 3. Readiness for Advanced Self-Health Management r/t expresses desires to manage illness a/e/b keeping hypertension controlled. I am mostly worried about the last two, not really sure if they are a priority diagnosis or if they should even be a diagnosis for this patient at all. I wanted one that would cover the right flank pain, and another that would cover her risk for developing diabetes (and the fact that she has hypertension) because this patient IS obese. I just don't think number 2 and 3 fit. Any help would be appreciated! Thank you. :)
