  1. I need the same advice. Did you find your answer? I still chart every two hours but add assessment on skilled nursing form
  2. It does make sense. It is medicaid so it is every two hours but I guess I can put pt in crib with eyes closed etc for those. I also have a skilled care sheet which has peg care, trach care etc so can I just leave those out now? My boss changes things around if she gets an idea from someone who works at the car wash it seems so i never know whats right. i know to chart vitals on both or at least i have always done. She sent a text yesterday that said to only reflect fresh entries and not to duplicate any past entry so she must mean daily but my meds are the same asmostly tube feeds through pump. UGH. Thank you so much for any of your feedback and what all you have given me. yall are the best.
  3. I have been a home health nurse for a few years and love it. I have the same patient for at least 8 hours a day. When I started charting I did it q two hours but then I went to summary notes. Now we are back to at least every two hours. What times do I write the notes? Do I write it using the time I start or did it or am writing it. I pile a bunch of stuff into one block so I woud have a million times if I broke it down. Ill paste a copy too bc it may be all wrong. My boss who is the owner of the company is not a nurse and has no clue what she is doing. I type my notes and am trying to make sure the one I wrote Friday is ok bc she said Medicaid came in and checked my chart and asked to see the last week. I am really hoping she is doing this BC it is time for a new Medicaid care plan. i wont be able to post times but each paragraph is a time and the times are like 1100 which is when i get there and then 1220 and 1300 and meds are at 1400. Any help is greatly appreciated. Thank you so much. This is actually 1100 t0 1900 and those are my shift hours. It just doesnt seem right. Do 'i set apart each thing done with time? im soooo used to summary reports. Thank you so much.
  4. Hi! I saw a site on here once that was a dentist that used to be a nurse and had great legal advice. I didn't save it because I was told my case was dropped. I was first found for medicaltion diversion. After passing a drug test 4 times and going to the Addiction Center and RPP who all deemed me not having a problem and dropped my case I get a letter from the LLR telling me I have to reply by friday and accept the discplinary action or have an attorney. Now they are getting me for improper medical conduct I believe bc I signed out two fentanyl patches and only had one witness. I can't even remember the details of this now. I don't know what to do. THe action is pay $500 and then take 5 classes online in 6 months but it will still be on my record. My husband wants me to just get an attorney. I am so scared.
  5. I took the NCLEX RN today at 2:00. I am getting the good message but am a nervous wreck! I had 75 questions and they were also mostly SATA and prioritizing ones as well as tons of meds. I do think that if I failed it would have taken more questions to figure out I didn't know enough to pass if that makes sense. I usually test well and was thinking I was borderline the whole time so I figured I would get more questions. I sooo hope this message is true! I have tried so many times now and need to be patient and wait. I am afraid if I try to many times it will put me through to the cc# info! This is gonna be a long 48 hours!
  6. :idea:i am in desperate need of help! we were just told we had to write essays for placement for our practicum (72 hours). they must be in before sept. 15th so that gives me a day. i am sooo not good at these kind of things. my instructions are to: describe why you have an interest in the area you have chosen for your senior practicum. students usually only get one of their choices if there letters are great. i have no clue where to start. i need several letters. nicu (main), mom/baby, labor and delivery, er, picu, or. they all go to different areas so we were instructed to write letters for all areas interest in. i narrowed it down to 4 with nicu being primary. nicu: i have always wanted to work in the nicu. i thought my mind had changed when i had a baby there for 2 months but it only made my decision stronger. i enjoyed clinicals there so much. mom/baby, labor and delivery: i am not sure if one is needed for this one or not however it is one of my top ones. i have always wanted to be a high risk l&d nurse or nicu. er: we didn't get to do an er rotation for clinical and i think it would be a good experience. picu: love peds as well as critical care. i have also had a child in picu and it just holds a special place in my heart. or: enjoyed or rotation. lots of learning experiences. any help would be soooo appreciated! i have looked so forward to this part and now we find out that a lot of the major colleges nursing programs end at the same time my class ends (we have classes finished every 5 weeks). we have to compete with these students as well so placement is tough and the letters are important and are making me sooo nervous!!
