you don't have to....its in your record of nursing program already, and your school is required to fill in those stuff....

@retetestis..i mailed my declaration of nursing education and record of nursing program to the BVN july 9, i phoned them a week after, and the BVN evaluator told me that it takes 6-8 weeks for them to response...so, its been a month now haven't receive any mail from them, yet i hope that it is my ATT that i would receive this time..

hi bluecross, how are u?..did you send your record of nursing program to them and they lost it too? that happened to me also, they denied that they have it...so, what i did, i processed again from our school, good thing i have my mom their..and the school was very cooperative, it was done like a day and my mom send it to me afterwards....and then after 2weeks i received my record of nursing program, and mailed it to the BVN immediately along with my declaration form..after a week i phoned the BVN evaluator and ask her if they received my record of nursing program and declaration form(just to make sure that they have it now this time), and she told me that they have it already, and i should wait like 6-8weeks for their response..so, it was a month ago now,and i have 1 more month to wait...im just hoping then that everything will be fine, and i would receive my ATT this time.....

When God closes a door, somewhere He opens a WINDOW..stop being stuck behind that door...go get up and find for a bright window,..our life not ends in that closed door,..its only the beginning for a brand new start.....GODBLESS TO ALL.

don't send your license, just a copy though.....

@proudpinay,.,yes I was able to complete all their requirements needed for the application, I submitted the sealed envelope that has TOR, Record of Nursing Program, and RLE in it,...and I was surprised that they state in the letter that the Board is requiring me to have them my sealed Record of Nursing Program as to continue my application process,..I was thinking then probably that they just overlooked on my papers, and they also state that they had received my Transcript from Registered Nursing Board,..but they need my Record of nursing program to complete the process as well as the declaration of nursing educ...anyway I should call them tomorrow for any clarifications on why they didn't have my record of nrsng program, and to ask them about the declaration of nursing, i still don't know what to answer though,...thanks proudpinay, ill keep u posted on what info i may get tomorrow..Godbless to all..:)

Hi guys, I just received my mail from the Boards today asking to complete the declaration of nrsng education form, yet I don't know how to answer #5, ....If you guys don't mind helping me, I was wondering if what are your sources in answering # 5 **Date started, **Date completed, and **Numbers of Hours Worked. I was thinking If do I need to call my school for that? And for #8 is it ok if I would send them a photocopy of my expired PRC card and some photocopies of certificate that I passed the NLE in Philippines even its not certified true copy?....any response would be very much appreciated....THANKS..

hi melai, am willing to help u but can you tell us if were we can find the form of DECLARATION OF NURSING EDUCATION or/LICENSURE? i called the BVN a day ago in the guy told me that i would receive their response this july and i guess i would have the same response like u had......and, actually, the BRN only transfer the copy of our TOR to the BVN not all of our nursing record though..

yep, am here in san jose, i heard the ccsf but i haven't tried it yet,.. i read ur post while ago and there some requirements needed to apply. definitely, i have no problem with that, as long as they can cater us, that would be really nice tho, but am wondering about the "space-available basis", do u have any idea if how long we gonna wait for that?..thanks..

hi pogingnurse, have u tried to ask the BRN if its ok to take those subjects on ur school back in the philippines?... just wondering if our school in the philippines could cater us with our deficiency, i guess thats would be great,..

@che_guapa yes they requires me to complete additional course work, both theory and clinical practice in OB and MS..

@che_guapa how about ur RLE checklist, maybe u have still those..actually, that was i did, since id lost mine too, so, i photocopied my clinical cases from there, and have it signed as a certified true copy by our dean,.unfortunately, i was still denied by the CABoN..

@retetestis did your analyst told you to pay for the $30 service fee?..my analyst did not mention about that, i tried to asked him but he answered me to just send him a written request, i don't get it though,..now I'm a bit lost, since, i already mailed him the written request..
