  3. Hello Nurses, Hope you can help me with this. I had a situation where I worked one of my PRN jobs about a month ago. And I had lots of charting to do. A couple of weeks later I realized that when I charted on one of my residents I put continent of bowel and bladder but in reality has a colostomy. What do I do to fix the situation? It's almost been a month I last worked there! Should I do an addendum? I know I do a lot of overthinking but I don't want to lose my license

  5. Yea I charted I'n the nurses notes I'n her chart and we document it I'n a notebook that all the nurses write I'n on what happened on the shift so we could know what happen on the previous shift! It's like a cover urself notebook so there for subverted being said what u told me is absolutely true!i will check te chart when I go back to work I'n a few days because I work a weekend special which we automatically get our 40 hrs! I only work sat and sun and I love my job! I hope everything works out for me! Thank you so much!
  6. So I shouldn't bring it up to the DON being that it's too days later? Cuz I'm thinkin what if I say something and I'n return she says what made u wait two days later to question the order! So do u think I should just leave it at that. He was the on call dr! Any advice would help thank u so much! What would u do I'n this situation that happened two days ago. Would u just let it be.

So u think I should just stop over reacting and let it ride and if something would happen they would call and let me know the situation and I would explain what was said to me cuz I actually read back the order and he agreed cuz after I left she was I'n stable condition and that was 6 hrs after I gave it to her and I passed it on to the next sceduled nurse I'n report and she never questioned what I said and she's been a nurse for years. I'm just scared! I haven't received a call and it happened since sunday
  8. She usually gets a dose of 55 units of 70/30 and thats y I didn't really think nothing of it! But I thought about it because I'm a new nurse and I'm scared! This is my sec week
  9. The resident is ok. Nothing us wrong! The point us I'm scared and they never called me or anything! So I'm worried! I think it takes the dr 24 hrs to sign off the order and u font know if he did it yet! Should I call one of my co workers and see if he did or if anything is suspicious about the order! I'm loke scared and dont know what to do! If I should call and talk to my DON or what! Do u think I should?

  11. No I haven't told anyone! I didnt know who to talk to! I'm on days off right now! I explained the situation to my husband and he said don't u think if something was kinda fishy or if something happened to the resident they would call u cuz u were the one who signed the order off and u actually did read back the order! I never told anyone! I'm scared because what if try say y did u wait until two days later to voice ur concerns! The only reason I think I'm covered is because I read back the order to him and he okayed it! Do u think I should just let it be and if there was a problem they would call!
  12. Here is the problem. Ok i went in to check one of my resients blood sugar level and it was 425. So on the sliding scale it says to notify the doctor for anything more than 400 of course! So i called the on call dr about the blood sugar results and he said to give 50 units of regular insulin and recheck again in four hours. So I reread what he said, give 50 units of regular insulin and recheck again in four hours and he said yes and after taking the order i wrote RBVO per (doctor's name). So I gave her the 50 units and rechecked her within the four hours that was told and it went up to 435. I called him back and told him the results and he said give her 15 units of regukar insulin and recheck in 2 hours so read it back to him give her 15 units of regular insulin and recheck in two hours he said yeah one five just like the last time! And I was thinkin justlike the last time. What do you mean the last time you told me 50 units and I read back the verbal order. Does this mean trouble. I mean the first order I did read back and he agreed. The second order I read back and he agreed. What got me is when he said 15 units just like the last time! Im like what the hell is he talkin bout! Im scared! Any opinions or advice of what can happen would be greatly appreciated!

  15. When a c&s lab report comes back abnormal and the doctor ask what she is sensitive to and the nurse says macrobid. What does this mean? I didnt want to sound too stupid and ask the nurse what the Dr meant. Im a new nurse and want to be ready for the question just in case my patients culture and sensitivity labs come back abnormal and the Dr ask me what she is sensitive to and won't beable to answer the question! Thanks

  17. I studied the day before my test! Well at least not major studying. What I did was look over my lab levels signs and symptoms and tx, insulin levels, herbs, cranial nerves, and acid base signs and symptoms and tx! I think that's about it! Remember ABC with priority questions. And remember pain never killed anybody. So if u have a priority question pain will never be the person u go see first. It's always someone I'n worst condition!
  18. Your going to do fine! And if u do decideto go the Hurst way make sure u study the way she tells you because with hurst u have a 98% chance of passing the first time if ur studying it the right way. The book she will give u will show u exactly how to study! People that said they failed Nclex using hurst haven't studied properly and that's why they we're successful and she tell u that to! If u need any help with studying I can help u! It took me two weeks to study! Went take Nclex and passed! But if u have more time study study study your book with the videos and take the weekend for breaks and start again. I started with Saunders and was stressing out cuz it was overwhelming! Stopped and started with hurst and I felt wonderful! Ur never going to know everything! Like Marlene on hurst said if u haven't heard of it somebody else haven't heard of it either!
  19. U need at least 3-5 references. I used 2 of my clinical instructors and 1 of my class room instructor because she did clinicals with us also. So go get u one more!
  20. I would suggest doing Hurst review. You need to know your core content! Knowing ur core content you can answer any question! I was successful using Hurst Review on my first attempt! Seems like you need to change ur way of study and Hurst is the way to go! And I didn't answer thousands of questions! I answered approx 125 questions the night before because I knew my core content and I could answer any question! Good luck!!!
  21. I took the hurst live review! I suggest that u take the online course because u can replay the videos over and over again and that's whats going to help you the most! What I did is tool the live review and after the live review one of my friends bought the online review and we watched it til we could practically mouth the words as she spoke while going through the book with her!! It's fun and ur going to love it! All u need is hurst!! She will teach u how to think like a new nurse and that's what Nclex expects from you!! Ur going to do fine!!! I did it and u can do it to !!!
  22. Oh yeah and too add I sucked at peds also and hurst broke it down to perfection for me! If you do chose hurst the first thing your gonna say is why we haven't learned it I'n school that way! And yes there the best on med surf stuff!!they give you the main signs and symptoms NCLEX looks for because you know they could have like 20 diff signs and symptoms and your wondering how am I going to know that! I vote hurst hurst hurst!!

  24. I loved the hurst review! They have a 98% pass rate!! The only content you need to study is the core content for hurst review because all the other review books are too overwelming with info! NCLEX wants to know what your going to do as a new nurse and that's how hurst review helps you think! I'm 100% sure if you use hurst your gonna wanna give me a big hug afterwards! LOL!! I passed on my first attempt only using hurst and you can to!
  25. GOt my results In today and passed!! Here us how I passed! I did Hurst Review only!!! No other book!!! 125 questions from the Q bank and that's it!! I didn't do thousands of questions because Hurst doesn't suggest that! Once u know ur core content without a doubt you can answer any questions! I looked over the videos with the book they give you for two weeks and I was done! And I also prayed to GOD to strengthen me each day and take the fear out of my heart and put faith!! That's what helped me!!!
