I an working on a performance improvement project to reduce the number of unplanned extubations. What does your facility do? Do you use a tracking tool? Or some form of tool to evaluate the cirucmstances in which the unplanned extubation occured? I am looking for some form of tool to help track ours? I would appricate any and all help...

biblepoet: my suggestion for job shaddowing in an area of interest would be: be engaged. ask quetsions, interact with everyone in the setting. i do not know the type of setting you are wishing to clinically work in in the hosptial setting. however an individual whom is engaged with the co-workers he/she is shadowing with, will have the upper hand on someone that comes in and just walks around behind the assigned nurse. hope this helps and good luck with future interviews.
