Lost very important paperwork. Am I in trouble?
I hung blood today. Had another nurse witness and sign both our names. The type of blood, donor number and everything was on that form. Also the vital signs. Patient did well, had no reaction. I was in the room with the form when I got called to help another doctor with a procedure. Then the patients family came and the next thing I know is I forgot about the form and couldn't find it. Should I call my boss and tell him what I did? Will I be in a lot of trouble? The patient didn't have a reaction, not yet anyway. I then got an admission. Was very overwhelmed today.

whats the best way to take the heart rate?
I was hanging blood and the cna took the pre vitals for me. She attached the blood pressure cuff and it showed the blood pressure and pulse which were all normal. Then after 15 minutes I went in the room and took the vitals the same way. The pulse was showing up as 40. So I checked the radial which was about 45. 2 other nurses came in the room and helped me get the apical which was impossible to hear since the patient had wheezes throughout all lobes and major crackles from pulmonary edema. The telemetry showed that his pulse was in his 80's and he wasn't in any distress. Im curious how should I take the vitals next time so they're a little more accurate. Should I get the pulse from the telemetry monitor?

Why do hospitals care about health care costs?
I went many many years without insurance and I had no choice but to pay for my procedures. I got the bill and a due date to pay. No body babied me around. My simple blood work was 1,000 and as much as I didn't want to, I paid for it. I don't get how people not pay their bills. Its like buying a flat screen tv and then deciding not to pay the bill.

Why do hospitals care about health care costs?
People always talk about how such and such procedure is expensive and the equipment is expensive. Im curious why hospitals care about expenses when its the patient and their insurance who have to worry about their expenses? Also, if a patient leaves AMA, why do hospitals care? The patient still has to pay for their services with or w/o insurance if they decide to leave AMA.

What is an indwelling IV?
Never heard of an indwelling IV. Only heard of indwelling urinary catheter. Also, why is it preferable that the anesthesiologist sees the patient the day before surgery rather than the day of ? Does it have something to do with irritating the throat?

I had a work related injury in another state a month ago. They fired me. Since then, I am living in another state. I am due for follow up tests. How do I go about getting my tests done and making sure that my previous employer pays for it? They know I am in another state and they fired me months before so we dont have any contact.

I got a prescription for truvada after 13 days post exposure. Im confused because I thought these meds are toxic on the liver and kidneys and the doctor did not order any blood work. He ordered 20 doses which according to the cdc they recommend these prophylactics be taken for a month or until the patient comes out negative. This patient had an inconclusive test and will likely be tested in 3 months if that happens. My question is how long do you have to take the prophylaxis? Blood work? Lastly, Im having a drug test for a new job. They will also ask me what kind of meds Im taking during the physical. My mother tells me I cannot take this medication. They will think that Im a w&&&e and they will treat me differently

Im a new nurse in LTC. Was giving insulin with flexpen when I pricked my finger after injecting insulin. Took of my glove and milked my finger with blood coming out. Washed my hands and filled out an incident report. My facility did not have a policy on needlesticks so I continued on with my work. Two days later my boss read my indident report and after I begged her she had him tested. But she told me I was being ridiculous that these old folks dont have anything unless it states on their chart. Anyway, his HIV came back + but his western blot was inconclusive. I came to work and asked my boss to see his results, she said "dont worry about it. its negative" My co-worker told me to look at the results- she said that they're not telling the truth to me. My boss left the results on her desk and I went through them and even spoke to the lab director about them who explained that the western blot was inconclusive. My boss refuses to test the patient again. She says Im crazy and Im fired. She also told me that I waited an entire week to get my blood tested and thats against protocol. They didn't have a protocol for needlesticks until a week later they put one in!! She said she wants no part of it and Im on my own with payment. I had my blood tested now that my facility put in a protocol. But I have no insurance and my boss isn't paying for it because she said I waited too late to get tested. She also is mad at me for talking to the pharmacy because of violation of hippa. I feel alone. THere was no protocol when this happened and I feel I did my best at the time. I didn't know what to do and was scared about finances. Of all things, this patient had to test + This whole thing is a mess. The doctor prescribed meds for me but its been 12 days since the incident and Im without insurance or work!!!!!!!!!!!!! She also said she will sue my a@@ for hippa violation. If she didn't keep information away from me, I wouldnt have had to go through the results and call the lab

I am so worried. I was giving an injection to my patient when I accidentaly struck myself with the needle after injecting the patient. He has MRSA (which after reading about it, says its everywhere skin, blood) I made an incident report but I am so worried I will get sepsis. He also has cancer, pneumonia. Im worried about the mrsa. Should my employer offer me something? sorry meant to say needlestick not fingerstick

Effects of vasopressin on hrt rate?
I know that if you give vasopressin to someone with lets say septic shock, it will increase their MAP. But what about their hrt rate? To me it seems like the vasoconstriction increases perfusion but it unfortunately would lead to less blood blow to the heart and lowers the heart rate. What do you think? Im confused with the decreased filling time concept here

Noting physician orders
Charting is a world that Im lost in. Nurses notes are easy but checking the new monthly orders is hard. In my facility, you compare the new orders against last months. About 99% of the medication orders are unchanged except 1 or 2. How do I know if the doctor didn't add the new medication by mistake? Is there a way for me to know? Also, the lpn who writes the orders confuses me because she'll write a new medication order without writing that the other medication is discontinued. The patient was on 0.25 mg Ativan and the new order was .5 mg Ativan. She did not write that the 0.25 mg was d/c so I assumed the patient is not getting 0.75 mg. Thankfully someone caught it. The nurses who work with me day shift and evening are old and nasty. They refuse to teach me things when I ask for help. They just send me to do treatments which is checking blood pressures and weights, feeding the residents ensure, and doing wound care. We dont have foleys or suctioning or oxygen. They then criticize me and say things like, "you could have done that faster." or when I did wound care they'll ask the obvious like if I put on the 4x4 gauze. I left the Ensure in the residents room after she requested it stand for half an hour because its too cold for her to drink. When one of the nurses found it, she said, "where is that girl? I am going to twist her neck." I want to cry

KY nursing license by endorsement. How long?
Hi sherri I am also applying to KY. You need to ask them for a fingerprint card and send it back to them. As soon as they get that card back, you will be issued your temporary license to practice. Its only $20 more but much less stress. To get the real license, it could take some time. It depends on when they get all your paperwork.

She was yelling at me because she thought that I thought I was someone very special. And I really am friends with one of her friends. That wasn't a lie. She didn't say anything about the state.; the secretary did so Im not sure if it was even about that. But its over Im just going to forget about it. Thank you all for your help.
