  1. Is this the printed MDS that has been signed after submission? If it is it cannot be changed, but like you I am new to this as well. You cannot make changes to a submitted MDS unless you make a modification to a prior submission. Can someone else possibly help us out? Thanks. Doris
  2. :yeah:I agree with what has been said and when I do vital signs I also chart pain and use the pain scale in our facility rated 1-10. I have seen residents that run elevated temperatures, heart rates, respirations, and blood pressures because of pain. When you chart your vital signs and then pain as 1-10 this also gives a rational as to why one or all of the vital signs are elevated if no other symptoms are present. It is amazing then if the resident is having pain and a PRN analgesic is given you will see a decrease in the other vital signs. Doris, RNAC
  3. I am sorry to hear this about your mother, she sounds like a Nurse who like most of us are overwhelmed at times with patient case loads. Unfortunately, I have to agree with most of the advise. It is difficult for her to prove that she didn't shred any documents and that she did assessments on her shift. I agree that her best option is to get a lawyer that would be willing to work with her in regard to payment. I wish there was something else I could do for her but I suggest that she not sign anything until she talks to a lawyer. She could try to find one that may give her a free legal consultation first. There are a few lawyers out there that advertise for free consultation. Tell her to hang in there and to think before she signs anything!!! Keep us informed. Doris Nesiba RN
  4. Thank you so much for your help. I enjoy what I am doing but it is very overwhelming at time. It's great to have others to bounce ideas off of.
  5. Do you have Medicare Meeting on a weekly bases? Where you discuss reference dates, therapy, and discharge dates? :yeah:I am new to this site and a new MDS Nurse who is having difficulty keeping track of reference dates. I have the scheduler wheel and was also wandering if there are in other tools available. Thanks! for your help.
