im looking for how to say it, not what to say. i can write a book, but it does not look right.

with sweat pouring down, hands shaking while i drew it up and injected it. i was handed a hand full of syringes and a btl of flu vaccine in a nursing home. some how i got it all done. hint: dont forget to check where the tip of the needle is in the vial before you draw it up. have all the stuff you need. embarassing not to have the alcohol prep pads. dont forget to put the air in the vial. what other hints are there?

I wish there was too. Im a home health care nurse now, and I write like I'm writting a book. each nurse takes their nurses notes home, so I dont get to see what other nurses write. My RN supervisor told me the new nurse has perfect nurses notes. I really wanted to see them, but was afraid to ask. Ive seached to computer and did one CEU on documentation, that told me nothing about how to write them. A book on examples would be a number 1 seller for nurses. Wouldnt it be nice? How about how to chart for each specialty:))))))))))))

New to Nursing and I need some examples for charting. I am a Home Health Care LPN. The following is how I start out and a basic of what I chart. On duty, received report from mom. Client in crib, trach and G-tube patent and secure. Topamax 3-25mg. tabs crushed and dissolved in 10ml h20, given c 10ml h20 flush, tol well. Mist mask removed and Passy Muir cap placed on clients trach. tol well. Client occ. gasping for breath, if sidetracked, client will stop and laugh loudly. Client cont with Passy Murri cap, excessive drooling, productive cough, client pushs same out of mouth. oral sx for same. :oPROM completed, tol well with loud verbal sounds and laughing. Metoclopramide 3ml given with 20ml h2o flush, tol well. Back brace put on client, tol poor. removed same. Client ^ in crib with full assist from this nurse to play with big button toy. client stiffens arm but laughs loudly.
