  1. In Texas we cannot intubate. As a Paramedic / Nurse I can on a wagon while out in the field. Some said why would you want to?, because the faster we get oxygen to them the better the chance. If your working trauma an you have a couple of ICU's and ER and only one Respiratory therapist on duty (if they even let them, some do not), as well as not having a qualified Doctor there to do it then you want to be ready. As well you have the small hospitals where you have one Doctor on staff at night and sometimes they are in an emergency. There are many risks involved though some are Airway obstruction, laryngospasm, and aspiration as well as harming the vocal chords. The person that said nurses do not do it enough. True but how often do some nurses use their ACLS, especially at the larger medical facilities with more than one code team. It's all about training and mock drills. Education usually though barely have enough time to train and do very few Mocks on anything. So that person is right in that most nurses wouldn't do it enough not to cause harm. So why not have the necessary equipment to help out such as a light wand? Would it make it easier yes. I have seen Doctors though that have perforated going in with a light wand and other methods. I am one that thinks we should not be able to intubate. As well I think unless Respiratory therapy does it enough they should not either, it is best left to an anesthesiologist.
  2. Adenosine is a slam. As a 25 year trauma nurse I have given it many times. The thing I like about adenosine is it almost always works on SVT. Aflutter is a different story. Now I was a patient a week ago and went into SVT. I didn't even make the paramedic mad when I spouted off orders. He had the six mgs loaded and I said, no, do 18. I am a big guy and you will need it. I watched the monitor as he gave it. I went into a pause about a second later. I hate calling it asytole because if it was, then truly I wouldn't of seen it, or I doubt I would. Then I drop to about 100 and stayed their. There is a video you can watch of a sonogram while adenosine is being administered which is very interesting. Texas "(10) Other Drug Related--actions or conduct that include, but are not limited to: (A) Use of any controlled substance or any drug, prescribed or unprescribed, or device or alcoholic beverages while on duty or on call and to the extent that such use may impair the nurse's ability to safely conduct to the public the practice authorized by the nurse's license;" You can see that it states, prescribed or unprescribed and also and to the extent so there is interpretation there. Could it come back that it is and this and that and meaning and that. Don't take a chance. Too many laws leave interpretation in them and leave you hanging.   4. I have just built a review site for agency nurses. I started it in dallas fort worth, but Im more than happy if you visit it to input your agency and let you rate them. I will be building a hospital review site soon. I noticed there is not really one out there with reviews on them and I will market these to get reviews.   5. Don't know about CA. but in Texas L.V.N.s are dying here is my post at indeed.com Don't listen to anyone, listen to this. Yes they are doing away with lvns. Yes they have did this before but in DFW they are there. Yes you will always have a job, in crappy nursing home or a low paying Doctors office. Here is the deal. To get magnet status they need all R.N.s they don't care about you, in fact they will let you go but still bring you in as a agency nurse. This way they can still say we don't hire L.V.N.s. To go one step further some facilities in DFW have went all BSN's so the adn's are getting it now. I wish there was some way we could sue or make the state do away with L.v.n.s and test us out for R.N. status.
  6. I have your answer, it is not so easy to get hired but there are more out there and just keep trying. Some services hire nurses to triage over the phone and pay very well.
