  3. We get pts post plasty or revision. Vascular marks two spots for us to cannulate. We just make sure we feel a thrill or hear a bruit before we cannulate. Luckily we were always able to cannulate and the pt goes home the next day. I had another pt who is a very difficult stick and whose fistula just never developed fully. Vascular kept saying its functional but even with a tight turniquet it was barely palpable. He ended up getting another fistula.
  4. Thanks for the reply .. A hepb neg ant (+core ab) running on a dedicated hep b machine raises questions for me even if we have MD order to use b machine...
  5. Question for my fellow HD RNs out there. What is the policy where you work with regards to patients with Hepatitis B Core Antibodies that are Reactive. With hep b Antigen positive patients, we bleach as well as isolate. But I have run into pts where the hep b core antibodies are reactive (but negative B antigens) and we use the hep b machine and isolate as well. 2nd question: Do you bleach your hepatitis B machine after a hep b antigen reactive patient? We do , but I have heard someone say it is not necessary to bleach the Hep B machine (because it is only used for hep B pts)..

  7. Hi all, i want to move to San Antonio, what is the job market like for an acute hemodialysis rn? I have plenty of experience, do hospitals contract out dialysis services? In Chicago we have davita and fresenius are the big ones. Would like to hear from fellow dialysis rn's if you're out there.
  8. I would agree with you, from my experience the issue of potassium going up because of fluid removal has never come up. I have never seen an overloaded pt with a normal K end up with an elevated K after dialysis using standard 2 K bath. And in almost every case, a person with fluid overload with elevated K (6-7 for example) will come down to normal K with 3,4 or 5 liters fluid taken off, thats from my experience, hope its helpful.
  9. Hi everybody, I've been a dialysis nurse for a while, mostly acute. Just wondering what your hospitals policy is on fresh frozen plasma administration with dialysis. We have a lot of coagulapathic patients in ICU and we are asked sometimes to give plasma with dialysis which we normally dont. PRBCs we give all the time. Ive asked around if any policy or studies regarding plasma and effect on dialyzer or pts but have had no clear cut answer. any advice will help.
