  1. OK. Thank you. So, it’s the sum of the piperacillin (80mg) and tacobactam (10mg). That’s where my confusion was, I guess. I didn’t realize you have to add the two to get the total mg for the Zosyn. Thank you again
  2. OK, I know, I apologize. Its so frustrating that I am not comprehending. I am old school and need it written out. Is there any way you can “show the work” on how you get 90 mg?

  5. Help. I can’t wrap my brain around how they got the dose of Zosyn for a baby. The order is to give 100mg/kg of piperacillin to baby who weighs 0.8kg. Concentration of Zosyn is 45g/ml. Each 4.5g vial of Zosyn contains 4g of piperacillin and 0.5g of tazobactam. The dose they want to give is 90mg of Zosyn. I’m trying to figure out the med math, but having a hard time understanding.
  6. I have been a NICU nurse for 14 years. Currently, I work in a Level IV NICU that is expanding it’s capacity and going to private rooms. The hospital has hired about 30 new grads to accommodate the new staffing needs. I know that it takes time for new nurses to acquire good assessment and general nursing skills, but I fear for the future. I’m finding that babies who do not get the proper nursing care suffer extra septic work ups, xrays, changes in the plan of care, etc. For example, if the CPaP does not have a good seal or if the ET tube is not suctioned properly, the baby shows increased work of breathing, then the doctor’s next move is to get blood or do xrays, when it’s a simple nursing skill fix. I try to provide education and tips whenever possible. Again, I know it takes about 2 years for a new nurse to become competent in such a specialized area, but it’s frustrating in the meantime. Any advice from experienced nurses on how to deal with the lack of good nursing skills from novice colleagues?
  7. Hi, My unit just started implementing annual peer to peer reviews. One question is “please provide your input regarding at least one opportunity for my personal and/ or professional growth.” Our into is a mixed population of senior nurses and brand new grads. It is easy to answer that question when referring to a new nurse. I am always looking for an opportunity to teach and guide them. My question is how to conquer this when it comes to a senior nurse? I know there is always room for improvement, but I do not hover over my colleagues, especially if they have been on the unit for 20 + years to look for an area where they need to improve. Not sure why this is stressing me out so much. Any ideas or input is greatly appreciated. Susan, NICU-RN, 17 years experience
