I work for a huge dialysis company that has clinics all over the US. We do monthly progress notes. If a pt comes in with a fever & ends up getting antibiotics & you chart that...THAT is the monthly note. If you have a pt that evrything goes well most of the time your monthly note may look like this: Pt arrived at HD as usual & without c/o. SBP up at 198 but comes down during dialysis. Fluid wt gains range between 2.5-3.5 kg/tx. No other complications noted. Pt does miss a tx on occasion. Hope this helps ! :)

In my clinic the policy is...whoever draws it up...gives it. PCT's are not allowed to draw up any med...such as heparin or lidocaine. They may draw up saline...at their pt chair from that bag. Different states also have different rules. I'm in Tennessee but i think this is mostly a company policy. It makes sense to me tho & I agree with it. Makes me & my liscence more comfy. :redlight:

to futureTNnurse: I have my AD in nursing from Chattanooga State. I am now enrolled in MTSU RN to BSN program...all on line. A distance nursing course & I love it! I'm just in my 1st semester & hope all goes as well as it is.

the antibiotics should be given by the dialysis nurse when the patient goes to dialysis

we use only 1 type of machine in our clinic & yes it will take a couple years before dialysis 'hits' you. one day it'll be 2nd nature to you. hang in there. dialysis is a very rewarding area of nursing

I also live in Tn & didn't know this law was passed. I am a nurse manager for a dialysis clinic & personally letting med techs pass meds scares me to death. I am working on my BSN & feel they don't have the theory to do such a task. I am responsible for all the staff that are under me & surely wouldn't want to get in a court case. If that happened the RN would be the one held responsible. ok that's my 2 cents worth.

As long as the patient is careful they may ambulate. I would however look & make sure it is stitched in.
