Thank you soooo... much for your replies. Heparin dose was 26 cc/hr at the beginning, it was increased to 32 cc/hr based on next ptt. I have another RN to check the calculation and signed. I piggybagged the heparin as a secondary with NS. DON said that the heparin did not go in, instead the NS went in and ruined pt's vein. pt's fistula was clogged prior to admission and that was a part of reason him being in the hospital. He has a perm cath for dialysis. pt is doing fine according to my coworker. By the way, DON said that HR is investigating. I don't know how much there is to investigate when pt is doing well. So, if I am being used as scapegoat, where do I go get an advice before hiring a lawyer. Is there a place inside of my hospital that determine the situation fairly? I really appreciate again for your supports. guys!

I am in my first year of nursing. Although I still make a little mistakes from time to time,I thought I was getting better until I received a phone call from my DON last Friday that I messed up pt's vein by piggybagging heprin drip with NS. She said that pt needs Central line because of my mistake. I programmed NS as primary at 30cc/hr , VTI 30cc, and hepain as a secondary 32cc/hr, VTI 500cc. I did not know that I was supposed hang heparin by itself. According to my DON, pt received NS instead heparin which ruined his vein. I did not know what to think of and I apologized to her that I made a mistake not knowing to hang heparin by itself. She told me that the incident is so severe that I either would be written up or suspended after investigation is completed. She left a message not to come back to work until I hear from her. I have not heard from her today. Although I apologized to her( at the time, I did not explained her what I did except admitting the fact that I piggybagged the med. However, I really don't understand why the pt received NS instead heparin. Heprin was hung much higher than NS. Pt is in ESRD with very fragile vein that Lab had to be drawn from his foot from the begin with. One of my coworker told me that lovenox is replaced to heparin drip. pt is doing fine. Would you give me some insight and how to resolve this problem? Any suggestion would be appreciated.

I am in my first year of nursing on med-surg. Last night, I piggybagged heparin drip with NS for pt with DVT. I did not know that I was not supposed to piggybag heparin. I came home this morning, my manager called me that pt did not receive his heparin and ended having central line caused by my mistake. HR is investigating about my mistake and for now i am suspended until the investigtion is over. I know this is a serious mistake. could I get fired from this? I thought that I was getting better and this is very frustrating. thank you for reading. I would appreciate for any thought.

Hi, IF I were you, I would apply as many hospital setting or nursing homes as a nurse aid. After putting a resume in, you need to be persistent with follow up calls. It may be a good idea that you visit manager's offices of the places. Take your resume with you. Consult with your instructors of your nursing school. They could help you to put your foot in the door. Mean time,If you already completed LPN course, you will need to take board exam for LPN. Until you get a job, you may want to get a financial aid via school such as student loan. I always believe if there is a will, there is a way. Best regards!

I also got stuck once. I reported the incident and the hospital drew blood from me and the pt. The senior nurses advised me for not to report. Their theory were that I most likely would not get contracted even though the pt might had HIV or hepatitis as long as I was healthy. That was the wrong info they gave. By the way, luckily, the pt did not have any of those diseases. Follow the protocol of the hospital. Although one is exposed to the HIV or hepatitis, there are preventive treatments as long as the one reports within 24 hrs. In addition, the incident did not interfere with my career. I don't know if this appy to your case. This is from Dr.Vicotor Frankle's famous book," Man's search for meaning," He consulted a pt who has fear of perspiring. He advised in order to cut this circle fromation. In the event that sweating should recur, to resolve deliberately to show people how much he could sweat. A week later pt returned to report that whenever he met anyone who triggered his anticipatory anxiety, he said to himself, " I only sweated out a quart before, but now I am going to pour at least ten quarts!' The result was that, he was able to free himself permanently of it by replacing his fear with a paradoxical wish. Hope this helps!!:wink2:

I was fired during orientation without 2 weeks notice. The reason was lack of competency. I thought that was the end of my career, but everything is behind and I found another job after 2 months. We learn lessons through experience. Prepare a strong resume and cover letter, start looking!! You will find one again!!

A interim manager came to fix the chronic problems (nursing care) we had on our floor,(med-surg), my manager was terminated. She started to letting people go. I am a brand new nurse. It has been 5 months since I started to work there. I really did not have a good orientation. The new manager knew that. However, A few weeks ago, I was terminated without given a chance to look for another job. The lack of competency was the reason. I feel like I was in a wrong place and wrong time. Now, Is this the end of my career? Will other hospitals even ask me for an interview? Any suggestions...Please!!!
