Hi, does anyone have any advice on how to disclose information about discipline on license to Davita? I have a shadowing day coming up but I did not disclose during interview. I really need this job. TIA

Im on probation, no restrictions, the only thing I need is a supervisor to sign quarterly reports for a year that's it, but soon as I attend an interview, it goes well but once I explain my probation, the DON is on board but once HR is mentioned its radio silent afterwards, its very frustrating, any advice (LPN in Michigan) please help!

Was it hard to find work during the probation period? I ended on good terms with my last job, they told me to apply again and they'll have to talk with corporate about my situation. I'm guessing it may be the same as yours (administrative probation).

OK, thank you. In the consent order, it stated that I complete 1 year probation regardless, so that's fine with me. How long until you heard back from the BON as far as reinstating?

Hello everyone, my monitoring program determined that I'm ineligible, what would be the likely outcome now if anyone can answer

Yes I have a migraine LOL, but not sure if you can answer this, but when the evaluator asks what info to send to the monitoring program, do I tell them to send everything or just the evaluation because theyre asking me to select a bunch of things to send, not sure if I have to send them everything like my whole life story etc.

Thank you, they're also requesting medical records that I have no idea how to access, and drug screen results, which I can probably use my app for. This is so overwhelming already.

Thank you guys again for all the information, I received the consent order and is starting the intake process, they deemed me high risk, so I have to start monitoring before the contract. Can someone explain the evaluation process, and do I have to continue ongoing sessions, this program already expensive enough.

Oh no, I'm sorry just to clarify by "dropped", I meant urine test, I remember that day as if it was yesterday. In the statement coworkers said I wasn't "myself". Honestly since the incident, I haven't drank since then, (wish I had proof of it). I figured I would have to do IOP, but my insurance and everything was cancelled, so I'm basically starting over with everything and living from paycheck to paycheck right now. How does the therapy usually works in the program? I can do it but my main concern is the cost, I've been reading about the therapy & that can be costly. But thank you, I needed that so much and yes, yes I will get my license back and this will be behind me!

Yes, they have evidence, I was hungover from the night before and dropped, even with trying to justify that, I know they only need one reason! Im waiting on a response from the monitoring program, website says once suspended, BON has to order me to enter. Now I'm just getting ready to send my response for the administrative complaint. Do you know what they will do afterwards of receiving my response typically?

Oh yes the stories I've read and heard! I've gotten consultations from lawyers, but they were all insinuating that ill be placed in the program, and what they'll basically do for me is speed the process of dissolving the suspension quicker and accompany me to a hearing if I was to have one. But how abruptly my license was taken, the money I had saved, I had to pay bills, I couldn't do both.

I've contacted them, waiting on a response, once I checked the website it's saying that once the license is suspended, I have to get ordered from the BON now. I wished I had known about the program and the self reporting after the situation, I would have started immediately, but that was my fault not doing the research until after the fact.

Thank you for responding, I called and emailed, hopefully ill hear back from them soon but as I checked out the website, its saying that since im under suspension, the BON would have to give an order for the monitoring.
