One thing I learned is to remember to go over charting the weekend prior to the surveyors coming in on a Monday. I had issues with two well seasoned RNs charting poorly (had training provided in staff meeting to help them) however one of them charted on two residents that made it look like abuse. The charting was done late Friday and Saturday, Surveyors were on site within an hour of me showing up for work the following Monday. I made the mistake of not going over the weekend charting. It was the worst survey I have ever had and because of the charting it resulted in two serious tags. The charting involved dry skin on one resident and an old healing bruise from a fall on another (fall was documented but the way the RN charted it sunk us). Anyway, I lost my job over it. The best advice I can give is watch those nurses who cannot chart properly no matter how much training you provide. You as the DON will take the fall. Learn from my mistake.
