    Usually my assessments stay the same, I write note in that one can depict how that day went for the resident. I include medications I given to them that day, note no side effects, etc. Their mood, any visits or activities they did. Because when I read therapy notes I see exactly what they did that day, so I try to write the residents day so when someone reads it they kinda know what the resident did that day r/t nursing
  2. Mds does care plans Social service sets up transportation for appointments Unit manager makes appointments for residents, and fix orders, pharmacy recommendations, have time to audit floor nurses but can't lend a helping hand Floor nurse does everything else.
  3. LTC are frowned upon mostly due to short staffing of CNA/nurses which gives a high patient load to the staff from what I know. Also good management is key to a LTC facility. I believe those who otherwise frown upon LTC nurses don't really know what we do and how SNF works. They see short staff and soaked residents but don't know all the other responsibilities and charting we have to do.
  4. I work snf side 2-10pm, have about 12 short term and 5 long terms. Once u start working with them and getting to know them, the medication pass gets easier. Usually schedules at 1600 and 2000.
