  1. I am an LPN as well. I completed 3 years of monitoring in Arkansas and wanted to go back. I checked almost every school there with an LPN to RN program and the answer was no. When you're in monitor your license is considered encumbered. Once you're done it will be unencumbered and you can attend. Your license will always show disciplinary action but if you complete monitoring successfully it will be unencumbered. I am currently doing my RN prerequisites now that I'm done with monitoring to go back and get my RN. You can still work on RN prerequisites while in monitoring just can't start an actual RN program yet.
  2. I got to pick who to give contact info for. I'm in Arkansas btw. I wasn't working as a nurse but I know one I picked was a current supervisor.
  3. I had to provide contacts however my evaluator did not contact any at all. However I did have 2 people write letters regarding my progress that I brought with me in a sealed envelope with their contact info. One was my AA sponsor.
  4. My next step would be to mail a certified letter.
  5. Did you submit your letter through the nurse portal and not affitiny? That's what I did and it did take about a week I think. I also sent messages through affinity as well.
  6. I feel like you need to self report this to the board of nursing and they will want you to enter into a diversion program. In my state legal charges have to be resolved before you can resume practicing again. Sending love and prayers.
  7. Thank you! There were no special requirements. Per my consent agreement I had to I submitted a written termination of probation request. I typed it up signed it and submitted it via the ARSBN nurse portal. I submitted a last personal report and employer evaluation through the Affinity app. You still have to continue to check in until you get an "Action Cleared" letter. I got it through the nurse portal account. I had to check in a few days after my completion date until I got the letter and was no longer able to log in to Affinity. Congratulations on completing monitoring!

  9. I am a recovering alcoholic and I always regret not getting help sooner. You should see if your employer has a EAP. You also can't be fired for going to rehab. It's against the ADA.

  11. Hi everyone! I just finished 3 years of monitoring in Arkansas and am so proud! I am an LPN that wants to apply at other places now that my license is unencumbered. I'm a little nervous about the background check. I can pass a criminal background check but 8 years ago my name was placed on the adult maltreatment registry while I was working in LTC. I came into work impaired and failed to administer meds. I know there is no way to get off this list and I've taken responsibility for this. My question is where does this exclude me from working? I work in an outpatient setting but im wondering does this exclude me from working in hospitals or going back to get my RN? Thank you!
