New nurse, opinion?
Would like to know what others think inthis situation I am a new RN, second week off a 12 week orientation on a Heme/Onc floor. i felt on top of it tonight. Saw my pts PCA bolus order was expiring. She needs/wants it on the dot q4hrs. She was due again for 6mg bolus of morohine at 615. At 3 am I page don't the intern to let them know that I needed a new order placed for the bolus dose for my PT at 615. I told him she gets it every 4 hours on the dot he comes back to me, can't figure it out. Tells me he's working on it. So I wait. He puts in a some orders that don't make sense. So I page him again. Asking him to please get this in. I believe my PT is in pain. I also know AM shift is coming in and I try REALLY hard to give a good report and to be able to do as much as I can before handing my pts off anyway, AM nurse, try to update her on this PT (she's had before) she's not listening to anything expect the fact that the bolus dose isn't in yet. I told her I paged many times and that the MD is aware and is working on it. He came out and actually told me he was going to ask day team for help putting the order in. anyways the AM RN is bossing me around telling me to frantically page pharmacy asking for help and then tells me to follow her as she barges in the physicians room to try to get an order. It's 7:20 now. And I have 3 more reports to give, on pts who have other things going on besides an hour late bolus of morphine. I know my PT is ok, she's got a basal of 0.5 and PCA dose of 0.8 every 30 min. She's a leak PT with abdominal pain from colitis. the AM Nnurse is literally Frantic about this, and will not let me handoff until it's resolved. What are you thoughts?

I am a new nurse. Still learning and afraid of getting in trouble or hurting my patients. My patient has been refusing meals. Given ensure with some encouragement and had dinner time bs sugar of 269 Orders were... Humalog 5 units after meals And sc humalog - so (from what I remember, 3 units? Is what I gave I held the 5 units due to her not actually eating anything. She took maybe 1/3 of the ensure. My preceptor says I was wrong and should have not held the additional 5 units. What do you all think? I am learning and just curious what others would have done

Being a new nurse sucks
I am a new nurse on a VERY busy oncology/ stem cell transplant floor at a large teaching hospital. I graduated and February and am now on my 2nd week of orientation on days. I prefer night shift. I worked as a tech on A surg floor for a year and half while in school. i feel dumb and stupid. I don't know so many things and preceptor is great and very nice but she assumes that I know a lot of stuff. When in reality I have no idea what she's talking about. She gives me this crazy look like, omg I can't believe you don't know. For instance, what a 'male bed' is. I don't know that shared rooms are only the same sex. I wasn't trained in shared rooms and I had no idea. I'm not used to residents and interns. I'm not used to epic. I don't know anything about chemo other than personal expierence from helping my grandma and dad, who both died over this summer so that has been hard in itself. my hands shake when drawing up meds and I'm not used to all these protocols and IV infusions...I'm not used to central lines and ports and constant MET calls for sepsis. Constant orders for fluid replacements and not to mention I must be chemo certified in 6 mo. I can't even get basic IV infusions and tpn lipids etc down right. they only teach you so much in nursing school. I did an ADN a program, so I'm not sure if that's to blame or what... i iust just feel so incompetent. I hate it. And his is not what I thought it would be like. I feel like I'm completely lost and disoriented. I'm wondering if maybe I should apply to something slower paced like rehabs, or outpatient settings. I can tell my preceptor thinks I am slow and inexpierenced I am 2 weeks in and I am miserable. I'm not sure if I should give it some more time or if I should start looking for other jobs.

Night Shift and Dogs
I am a new grad who has been oriented to days and I am beginning night shift soon. My preferred shift! Yay. my dog has been staying with my mom while I was on days. I felt it too long for him to be alone during the day. My dog is used to me working 11-7a as a cna. I'm sure he will do fine, so please no comments on how i shouldn't have a dog etc. he sleeps all night long and I know he will be ok. im just looking for tips and advice! Thanks

First day of unit orientation
First day of on unit orientation as a new grad. im so nervous. Should I be there earlier than 630? I don't even know where to clock in im nervous about my preceptor assigning me my own patient tomorrow. I've heard mixed things on whether or not the first day is shadowing only or not. i don't know if I have access to epic yet or Pyxis. i don't know when to chart. I don't know a lot of things! Im so scared and I'm honestly dreading. I want to be excited but this is so overwhelming

Gross Things Patients Do
I once had a patient who... not sure if he really needed to do this or not. By looks of it, did not need to...anyways, was used to literally picking his own feces out. He asked for help and instead of having a normal BM, would reach down and pick his own feces out. And then throw it in a bedpan. Bare handed. He reached down, picked it up, then threw it in the bed pan. Stool was wel formed and appeared to be moist. Not the typical stool thats impacted...He was a normal dude too... wasn't my patient and it was during a rotation. So I didn't follow up or find out... situation or background, but wow...
