  1. I work in a nursing home as an MDS nurse. Friday I arrive and immediately the floor nurse needs me to draw labs from a picc. Only RN can draw labs from a picc at our facility. So as the only RN in the building besides DON I go draw the labs. It all goes well. Now today I happen to be reading through the orders and notes and to my surprise the picc I had drawn from was newly placed and the xray confirmed it was in the right atrium with no complications. But they had called the picc company to come out and fix placement. So turns out at the time I was drawing labs and yes flushed it with normal saline they were awaiting for the placement to be fixed !! Now I feel like I did something I shouldn't have. How big of a deal is this ? I guess I literally have to double check everyone's work before I go do one simple skill.
  2. I have 2 questions. I have an lvn and rn active license in Texas. It's time to renew in april 2019 and they are both due at the same time. I know I would have to do 20 hrs for lvn renewal but this will be my first time to renew my rn do I still have to do the 20 cne hrs? Second question. I am taking time off work for a while to have a baby and I won't be going back until April 2019 to work. Can I leave my rn license active or do i have to make it inactive? I read the Texas board of nursing website but i still have questions. Thanks
  3. So here is my situation. I found out i was pregnant in may and was working full time on a medsurg floor 12 hr shifts very busy. Before i found out i had planned on quitting this job soon because i was unhappy in the job. Anyways, at 6 weeks pregnant i went ahead and quit as i was starting to suffer from all day nausea and vomiting and already knew this job would be beyond my limits since i was struggling with it already. Now im 12 weeks and starting to feel better, i want a part time job to bring in some extra money. I do have enough in my savings to live off of until after the baby is born but i dont want to use it if i dont have to. If i go on interviews should i tell the employer im pregnant? I am not quite showing yet just look pudgy like i have been eating a lot. Please no judgement on my situation.
  4. Patient load and no help. I like the patients but its just becoming too much for me. I cant get one person to help! And they constantly pile the work on and on! 12 hr shift turns into 14 hr shifts. Plus when i have to stay late it stresses me out i already have to drive nearly an hour home.

  7. It's 40 meq iv over a few hours with ns runs at like 25 ml per hr and the ns at 30 i don't remember exactly off the top of my head
  8. The potassium bolus protocol is ordered for below 3.5
  9. So i took care of a pt who when i came on to report his potassium level was 2.7, the night nurse gave him 40 meq of potassoum tablets and then it was rechecked at 10 am on my shift. It came back at 3.4, so i wrote a note in the chart for the drs that the level came back this morning at 3.4. Then i forgot about it the rest of the day. When it was time to give report the on coming nurse asked me why i didnt give the potassium bolus! She said she knew i knew about it because she saw my note left in the chart. I must have overlooked it. The potassium bolus shows up in the prn medications list and i must have acknowledged the order but i didnt even think to do it. It was a very busy day as this is a medsurg floor. The night nurse said its ok she will give it. But just the fact i forgot about it and her asking me makes me feel so dumb. I usually dont try to leave work from my shift for other ppl. I just feel guilty. Am i making this too big of a deal? Its really bothering me.
  10. Hello. I've worked in nursing homes for 3 years and now am new to working on a medsurg floor in a hospital. I'm still in training with a preceptor. At this point I've been allowed to take care of up to 3 patients. My problem is this : I feel like I know nothing. Iv stuff and all the skills are new to me since I've never done them in a nursing home. I already feel discouraged because I've made so many minor mistakes . I feel maybe the hospital isn't for me after all. I'm not sure what to do. Did any of you feel this way?
  11. I did a picc dressing change and after I removed the old dressing I couldn't get the blue bio patch off the patient it was sticking to their skin so I used my hand to grab it off. I then put on the sterile gloves and cleaned the area. As I was using the tweezers to put on the new bio patch it slipped out and landed on the patients gown I quickly picked it up. I feel like I should have got a new one but I didn't and now I feel guilty! I'm worried the patient will get an infection and it will come back to me. I'm new an still in training but my preceptor let me do the dressing alone. Any suggestions ? I'm worried.

Yesterday working in a nursing home a family member approached me and rudely said why has a set of vitals not been take on her family member who happens to be actively dying and on hospice care. She said this should be a priority patient and griped at me up and down about it. I didn't know what to say accept that I had not gotten around to it yet. There was no indication that I needed to take vitals, other family was at the bedside since I arrived on the shift. I entered the room 2 times to check on everything and the patient had no signs of distress just lying there quietly. Am I wrong for not taking vitals on this patient?
  13. I'm starting the night shift this Friday how can I adjust my sleep schedule any tips?
  14. Should I use Kaplan or Uworld for the NCLEX-RN to study? I would like to pass on the first time of course. My class is giving us the Kaplan review course and today was day 2 of 4. We have utilized kaplan material all 4 semesters and it seems so hard to me and the answers to questions feel so bizarre. I hear a lot of good things about Uworld. Which one will help me do better on the NCLEX in your opinion?
