Process Regarding Request for a Fingerprint Card in NBI :-S

Hello, fellow nurses! One of the requirements to register at Michigan BON to take the NCLEX-RN exam is a request for a fingerprint card. Since I'm here in the Philippines, a friend told me that I can request it at NBI. I have some questions regarding this process:

1. In which branch of NBI where will proceed to request for a fingerprint card?

2. Once I visit the specific NBI branch, what will be the process, what requirements or things will I bring, and what fees will be collected?

3. How long the process will take and how long when can I claim the fingerprint card?

Please help me, my fellow Filipino nurses, especially who have recently applied for licensure by examination in their preferred US states that require a request for a fingerprint card. Also, please answer my questions seriously because I'm planning to visit the NBI branch you will mention this Friday. You may reply on this thread or PM me. P.S.
