PERRL

Hi I am currently in LPN school. I have a really tough teacher right now that is not very nice to any of us. We were all taught to use the abbreviation PERRL in our nurses notes for assessment of the eyes. Well this new instructor has told us that you are not allowed to use that unless you actually measure the pupils. Is this accurate? I have 2 instructors telling us to use it and one telling us it is not correct. It is so frustrating!

My impression:I write PERRLA for most of my H&Ps. As you know, it stands for Pupils Equal Round and Reactive to Light and Accomadation. It does not imply a size in mm, but you are estimating pupil size bilaterally in order to asses them as equal. Again, just my two cents.
