Hi guys,
I am having trouble coming up with the answer to this problem.
You have a Morphine drip running at 0.8 mg/hr. The concentration of the drip is 2 mg/cc. How many ml/hr should the pump be running at? Therpeutic dosage for a morphine drip is 0.02 mg/kg/hr. The patient weighs 37 kg. Is this an appropriate dosage for this patient? The patient wakes up and begins thrashing about. The physician tells you to give the child a 0.1 mg/kg bolus. How many mg do you give the child?

I got the ml/hr the pump is supposed to be running at, which is 0.4 ml/hr.... I divided (0.8 mg/hr / 2 mg/cc ) to get the pump rate. I believe that is correct but I am having trouble determining if this is an appropriate dosage or not and how to determine the mg for the bolus. Thanks so much for helping me with this!

I have never heard of a pedi patient on a morphine drip.

you are being asked to determine a weight based dose for this patient. weight based dosing can either be order per hour (as this one is) or per minute. to calculate a weight based dose (per hour), i use the following formula: dose x weight.
if you wanted to determine the infusion rate, then you need to add an additional step and include the concentration. in that case i would use the following formula: (dose x weight) / concentration of the infusion.
when using any formula, it is important to remember that all units must be expressed in like increments.

although fentanyl is the primary analgesic infusion in the picu where i work, we occasionally use both morphine and hydromorphone.
