Remember Maslows Hierarchy of needs.

Physiological needs come first: food, water, breathing, circulation, etc

then move up

We were taught the Risk for diagnoses were last. I would place them in this order:

1. Fluid imbalance less than

2. Nutrition less than body req

3. Impaired oral mucosa

4. Skin integrity

5. Fatigue

6. Risk for infection

  • Experts

  1. deficient fluid volume (physiological need for fluid)
  2. imbalanced nutrition: less then body requirements (physiological need for food)
  3. fatigue (physiological need for rest)
  4. impaired oral mucous membrane (safety need)
  5. impaired skin integrity (safety/protection need need)
  6. risk for infection (anticipated need for protection)
