I've got this assignment which you need to explore a patients actual/potential problems.
the scenario given is:
A patient was admitted with an exacerbation of his COPD due to a chest infection. At present your patient is confined to bed due to breathlessness and he is currently immobile. He is breathlessness and therefore is eating and drinking /very little as removing the mask to do this means he becomes more distressed and anxious. He is dependent on nursing staff for his personal needs. The patient is using bottles or bedpans at present and he is quite upset about this. He finds it really difficult to use the bottle in bed and needs to stand up but again this is causing him more distress. If left alone too long he removes his mask and then becomes agitated and hypoxic and tries to get out of bed and he has not been sleeping well due to the noise of the oxygen and regular inhalers. Communication is difficult due to his breathlessness. He is completely dependent on his oxygen and his vital signs are out of normal range, in particular he has a high respiratory rate, high heart rate and high temperature.
so far I have outlined NUTRITION, BREATHING, ANXIETY AND DEPRESSION, EXACERBATIONS, MOBILITY AND ELIMINATION As being actual or potential problems any more ?? please
thanks
