trying to fill out a complete health history form for health assessment class and just not sure how to do this, Im doing it on a 92yo female Stuck at the Present Health or History of present Illness ---is this where i write the medical diagnosis???????? Help me Please

Medical Diagnosis of

1. L hip orif

2. Afib

3. clonic pseudobstrustion

PMH:

chf, copd, mr, Paraxysmal Afib-Ef 30-35%, Pulm HTn, cad, orthostatic HTn, H/o R pna and H/o MRSA, H/o UTI, H/o dementia, Asthma, anemia, B12 deficiency and osteroporosis.

it sounds like you are in the process of writing a care plan and are stuck with how to write up the assessment data. is that what is going on?
